License Renewal Quick Guide

Do you need to renew your license? Here’s what you need!

Before you can get a new or renewed license there are a few things you need to take care of.

Before you visit Client Services, here’s what you’ll need:

You need original documents not copies.

Make sure you fill out the application completely.

You need a medical history! You must complete side two of the application regarding a physical examination and additional medical history. The physical examination form must be completed and signed by a physician located in the Washington, DC area and it must be notarized.

Proof of residency: Make sure you review the documents you need to show where you live. It must match what is on your driver’s license.

You need to show your Social Security Card, Resident Alien Card, A Valid Employment Authorization Card, A Naturalization Citizenship Certificate, A Valid US Passport, I-94 Asylum, or a Green Card

Criminal Background Check: You must obtain a Metropolitan Police Department (MPD) Criminal History Request Form (PD-70 police clearance). The form can be obtained from the Municipal Center at 301-C Street, NW 1st Floor Room 1075, Washington, DC20001. Or visit the website and choose "Dept. For Hire Vehicles" from the drop down menu.

You must submit your driving record.

Clean hands and Business Tax Registration Forms: You must provide an original copy of your Clean Hands and DC Business Tax Registration Form. This form can be obtained from 1101 4th Street SW, Washington, DC 20019, customer service desk. Or visit online otr.cfo.dc.gov/page/certificate-clean-hands
